Radioactive Cobalt-60 teletherapy machines find a prominent place in radiation therapy departments India. The first cobalt machine SMS hospital (Picker Advance Tele Cobalt (ATC) C/9) was installed 1995. initial activity of the source 333 TBq (9000 Ci).In March 2019, output became lesser than 50 cGy/hr at normal treatment distance (NTD) 80cm which is clinically not acceptable for patient because increase time. decommissioning considered due to ageing and vendor providing services anymore. This present study describes safety aspect planning transport decayed disposal personnel dose measurements during procedure. Decommissioning began with preparation plan its submission approval by competent national authority. Radiation survey head performed before transfer operation AERB certified engineers. Then unloaded shielded container (flask Sl No 112) kept another outer (type B (U) package). It transported BRIT, Kota on receipt permission fromAERB.A done matrix format all sides this package.To measure doses, pocket dosimeter, TLD badge OSLDs were used. maximum received engineers .03mSv. Maximum level 1 m from external surface package found 0.83mR/hr. development new concepts practices field includes plan, actual operations as well safe management waste arising process.
